What is the Foaming Test?
Foaming Test is a COVID-19 Screening test performed with urine.
What to Look for with the Foaming Test?
Viruses; are infectious agents that contain their genetic structures, protein structures, and amino acid structures that are subsequences of these protein structures, which can only be seen with a microscope.
The virus that causes Covid-19 disease (Sars Cov-2) is an RNA virus with its genetic structure, protein, and amino acid structures.
Sars-Cov-2 multiplies rapidly as soon as it enters the body. In the meantime, some metabolic and chemical changes begin to occur in the body before clinical symptoms and lung findings occur. Some of these changes are blood, urine, etc. can be followed by tests. As the viral load in the body increases, the structures belonging to the virus, the metabolites of the structures belonging to the virus, and some substances formed due to the problems indirectly caused by the virus begin to be excreted in the urine. The rate of excretion is directly related to the infection ability of the virus, the number of viruses transmitted to the individual, the strength of the person’s immune system, the person’s metabolism, and other diseases he or she has.
The reactant (MSK), contained in the Foaming Test and specially prepared in the laboratory, reacts with some substances belonging to the virus and their metabolites, as well as with some substances caused by the virus, which should not normally be present in the urine, and the urine in the test tube foams up. The foaming rate increases in direct proportion to the viral load in the body. People with a low viral load will have less foam, and people with more will have more foam. As a result of the data obtained in laboratory studies in patients with a diagnosis of COVID-19 and healthy individuals who have not had COVID-19, a color scale was created on the test tube. Foaming test results are evaluated according to this color scale.
In Which Situations Can Foaming Test COVID-19 Screening Test Kit Be Used?
1- It helps to identify people in the incubation period who have some clinical findings, but whether they have COVID-19 or not, and to determine their clinical status.
2- It helps to follow the clinical status and prognosis of people diagnosed with COVID-19. It helps to carry out the daily clinical follow-up of people whose prognosis worsens when their prognosis is good, or people whose prognosis is getting better.
3- It helps to distinguish people who have similar complaints but have another infection that is not COVID-19.
4- It helps people who are suspicious of COVID-19 and in the follow-up process, who are in the quarantine process, who have completed their treatment and who are in the follow-up process at home, to learn their clinical status practically, to share them with the doctor who follows them, and thus to make a quick decision about the treatment process.
5-It helps people who are worried about COVID-19, who have to enter a suspicious environment and who are uncomfortable with this situation, to control themselves and thus to apply to health institutions quickly if necessary according to the test result.
6- Apart from COVID-19, it helps people who have some substances in their urine that should not normally be in their urine, to check themselves and to consult a health institution in the early period.
7- It facilitates the work of healthcare professionals in the pre-diagnosis, definitive diagnosis, and clinical follow-up processes.
How is the COVID-19 Test Used? Urine Sample Collection and Use of the Test for Covid-19 Scanning Test Kit Use:
1- Sufficient urine sample is put into the urine container. Since the substances and metabolites of SARS Cov-2 will be excreted in the urine according to the metabolism of the individual and the severity of the infection, they may not be present in the urine at the same concentration every hour of the day, especially in non-severe cases. For this reason, the most ideal sample for individuals to test is urine that has been in their bladder for at least 1 hour. This example will be given after getting up in the morning 2. If it is not possible to change the catheter, the urine in the existing urine bag should be mixed very well and the sample should be taken.
The first urine after sexual intercourse should not be used for the test.
2- Remove the label on the urine container.
3- The cover of the Foaming Test tube is carefully pressed in such a way that the hollow part in the middle comes to the pointed area in the hole on the urine container.
4- With the effect of the vacuum in the tube, the sample in the urine container starts to fill into the test tube. It should be checked whether the urine reaches the level of the black line on the tube.
5- After the urine filling is finished, the test tube is carefully removed from its location. The vacuum in the tube may rarely cause the amount of urine to be below or above the black line due to the altitude where the test is used.
In such a case; The test tube should be placed in an upright position with the cap on the bottom, if the urine level in the test tube is above the black line, the test tube with the syringe in the Foaming Test box. Some urine should be drawn until the level of urine in it reaches the black line. If the level of urine in the test tube is below the black line, it can also be injected through the injector. Some urine should be taken from the urine cup and urine should be added until the amount of urine in the test tube reaches the black line. The plastic hollow area on the cap of the test tube is the area where the needle will be inserted. After this process is done, the test can be continued.
6- The tube should be shaken well for 15 seconds as described in the promotional videos, and the reactant should be mixed with the urine. Shaking time should not be less or more.
7- By gently moving the test tube left and right, up and down, or by making small taps on the top of the tube with your finger, the foam that may have remained at the top of the tube is also allowed to descend to the liquid surface.
8- The test tube is kept on a flat surface for about 10 seconds with the lid side down and interpreted according to where the top level of the foam in the tube comes from on the color scale.
How is the COVID-19 Screening Test Kit Interpreted?
Green Zone: No viral load in urine. Positive clinical picture.
The Foaming Test result of the individuals who were diagnosed with Covid-19 positive and started their treatment in Green Zone can be interpreted as the treatment process was successful and the virus load in the body disappeared.
In an individual who has + results in the PCR test performed at the same time, but whose Foaming Test is Green Zone; In case the PCR may give false positives due to other reasons such as contamination and/or cross-reaction, the symptoms of the individual should be questioned in a full-fledged health center, relevant blood tests should be requested, lung tomography should be taken and re-evaluated by PCR.
Yellow Zone: Individual with a low viral load in the urine (and/or pathologies with the excretion of some substances that should not normally be present in the urine), a suspicious clinical picture, and an individual requiring follow-up.
*Individuals who need follow-up:
1- There may be an individual who has not been diagnosed with COVID-19 but has been infected with the virus.
2- It can be an individual who has been diagnosed with COVID-19 and is under treatment.
3- There may be an individual with an abnormal clinical picture, which goes with the excretion of substances that should not normally be in the urine, except for COVID-19.
In this case, it should be taken into account whether the individual has Covid-19 symptoms and the level of the foam in the Yellow Zone.
It will be beneficial for individuals who have symptoms and whose foam level is in the upper parts of the Yellow Zone to apply to a health institution by following the social distance, hygiene, and protection rules to take precautions in the early period.
Individuals who do not have symptoms should cut off close contact with family members and the environment for precautionary purposes, wear a mask, comply with social distance, hygiene, and protection rules, and be evaluated in terms of substances that are excreted in the urine and should not normally occur if the result of the evaluation is normal and 2-3 consecutive results in tests stay in the Yellow Zone or If advances towards the Orange Zone, it must apply to a health care provider.
It would be appropriate for individuals with a definite clinical picture of excretion of substances that should not normally be in the urine and who do not have symptoms in terms of Covid-19, to apply to a health institution about urinary system disorders.
Orange Zone: An unfavorable clinical picture with increased virus load in the urine (and/or pathologies with the excretion of some substances that should not normally be present in the urine).
Individuals with Orange Zone test results and Covid-19 symptoms; should apply to the nearest health institution by following the social distance, hygiene, and protection rules at the maximum level.
Individuals with an Orange Zone test result but no symptoms should apply to the nearest health institution by complying with the social distance, hygiene, and protection conditions at the maximum level. These individuals should be evaluated both in terms of Covid-19 and urinary system disorders.
Red Zone: Severe clinical picture with heavy viral load in the urine (and/or pathologies with the excretion of some substances that should not normally be present in the urine).
All individuals with a Red Zone test result should be immediately isolated, and should urgently apply to the nearest, fully equipped health institution by complying with the social distance, hygiene, and protection conditions at the highest level. All individuals in contact with should be taken under control under the same conditions. Even if these individuals do not have any symptoms, they should be evaluated in detail in terms of both Covid-19 and urinary system pathologies.
In What Other Situations Can Clinical Screening Be Performed with the Foaming Test COVID-19 Screening Test Kit?
Foaming Test Apart from detecting the virus load in the urine of Covid-19, it can detect pathologies in which some substances that should not normally be in the urine are excreted in the urine. In this way, if the test results are interpreted correctly, they can be used for early diagnosis and clinical follow-up. Tables in which there is intense excretion of various macro or micro-sized structures such as severe renal failure, diabetic nephropathy, eclampsia, some types of nephrolithiasis can be given as examples of these conditions.
For this reason, individuals with test results, especially Orange Zone and Red Zone, should be evaluated in terms of urinary system pathologies, even though they do not have Covid-19 symptoms.
WARNINGS:
The Foaming Test is a test that only shows the virus load at the time it is done.
It is known how contagious the SARS Cov-2 virus is. It should not be forgotten that even an individual with a Green Zone Foaming Test result is at risk at any time after the test. For this reason, it is extremely important to comply with the social distance, hygiene, and protection conditions at the maximum level.
